In order to stay alive, he is equipped with iron lungs inside his suit. The only time Vader’s face is ever seen is when Luke helps him off with the mask in the end of The Return of the Jedi. However, Vader was able to take his mask off provided he was in his hyperbaric chamber, but even then he used machines to help him remove the mask, not wanting anyone to see his face.
